    This tool is designed to support your academic and professional growth by exploring your individual needs as it relates to Sucess Coaching. Please be advised that request for transitions are not automatic, are never guaranteed, and are evaluated strictly on a case-by-case basis. Submitting this form does not initiate a coach transfer. Instead, it serves as the entry point to a rigorous, multi-step professional process requiring direct communication, a mandatory administrative alignment session, and an active 6-week trial period with your current coach. Approvals are considered only as a last resort after all collaborative remedies have been fully exhausted.

      In alignment with the program’s commitment to academic excellence and the development of professional competencies, the program establishes the following standards for the Success Coach-Student relationship:

      To ensure program integrity and equity, Success Coach transitions follow a structured, multi-step process. This process includes a mandatory 6-week alignment trial and dedicated action plans designed to foster collaborative resolution before a formal transition is approved.

    • Student Agency & Accountability

      In accordance with the Standards above, the student takes full ownership of their professional development and commits to navigating this alignment process with active accountability. During the 6-week trial period, the student will:

      1. Maintain the Schedule: Attend all scheduled coaching sessions consistently and arrive prepared for each. Implement Adjustments: Actively practice the agreed-upon communication strategies and feedback formats outlined in the action plan.
      2. Mandatory Check-In: Complete at least one substantive check-in meeting (preferably more) with the coach during the 6 weeks to actively work on the alignment goals.
      3. Required Follow-Up Action: Take sole responsibility for initiating and attending the final follow-up meeting with the coach within 10 business days of the trial's end date. The student acknowledges that failure to schedule and complete this meeting within the 10-business-day window will result in the automatic closure and forfeiture of the request.



       

    • Success Coach Responsibilities


      In accordance with the standards above, this transition review is entirely non-evaluative and serves as a logistical and developmental adjustment. To support the collaborative growth process during the 6-week trial, the Success Coach will:

      1. Provide Targeted Support: Deliver coaching and feedback utilizing the specific adjustments outlined in the action plan.
      2. Maintain Engagement: Remain available for the student’s scheduled sessions and the required mid-trial check-in encounters.
      3. Document Growth: Observe and note the student’s efforts toward professional communication and conflict resolution during the trial period.
      4. Participate in the Review: Attend the student-led follow-up meeting at the conclusion of the 6 weeks to objectively evaluate the effectiveness of the Action Plan and sign off on the final completion log.
