  • This form is designed to accurately track and document all attendance exceptions, including absences, tardies, and early departures. Your prompt and accurate submission of this form helps ensure proper communication and record-keeping. Please fill out the required fields with detailed information to assist in the processing of your attendance status. If you have any supporting documentation, be sure to upload it before submitting the form.

    Upon receipt of the form, the Director of Student Affairs will determine whether the absence is "excused" or "unexcused." A copy of the attendance form will then be forwarded to your professors for signature.

    Please ensure you provide a valid email address for each instructor listed in the course(s) above. This is required so that each instructor will automatically receive a copy of the attendance report. Forms submitted without the complete email information may delay the processing and distribution of your attendance report.
